The Someris platform

Six pillars.
Measured before marketed.

Software in this market is sold in adjectives. Someris will publish performance numbers only from its pre-registered superiority trial, scored against adjudicated clinical truth, with the scorecard snapshot cited beside every figure. Until that trial reports, this page describes what the platform does, not what it has been measured to achieve. The clinician stays at the center of every pillar; the platform multiplies what one coordinator can carry, with judgment and signature staying human.

[01] Extract

The whole chart, read in full.

Someris reads every page of the chart: EHR feeds, discharge summaries, therapy evals, and the scanned or faxed packets no text pipeline can see. Deterministic form readers run first at zero model cost; vision models read what the parsers cannot; and every extracted item carries its source document, page, excerpt, and date.

  • Deterministic form parsers (checkboxes, radios, GG grids)
  • Zero-cost deterministic extraction floor
  • Vision reads of scanned and faxed pages
  • Evidence provenance on every item
  • PointClickCare, MatrixCare, NetHealth, and FHIR hospital feeds

Scribes transcribe the visit that is happening. Record systems store the chart that already happened. Someris reads the existing chart into the payment assessment itself.

[02] Verify

Every draft scored before anyone signs.

A draft you cannot score is a draft you cannot trust. Someris scores every drafted item through a verification chain: was the evidence found, is the value consistent with it, is the code valid. Items that fail are flagged for human judgment, and an independent second pass escalates the genuinely ambiguous ones. A layer that does not clear its bar stays switched off.

  • Item-level verification chain on every draft
  • Judgment queue with precedent memory
  • Independent second-pass escalation on ambiguous items
  • Look-back window checks on dated evidence
  • Adjudication overlay of recorded verdicts

"Mostly autonomous" is an adjective. A precision number on a named corpus is a measurement. We will sell the measurement, not the adjective.

[03] Optimize

Every decision priced. Only defensible dollars.

Each coding decision is priced in per-diem dollars before attestation. Someris surfaces only revenue the documentation can defend: over-code guards, template-text guards, and computed-score integrity checks run against our own output first. When we disagree with a signed assessment, we argue both sides the way an auditor would, record a verdict with a confidence score, and keep the score.

  • Per-item PDPM pricing across all five components
  • Diagnosis recommendations ranked by financial impact
  • Guards against over-coding, run on our own drafts first
  • Assessment-date and primary-diagnosis revenue dials
  • Both-sides adjudication of every disagreement
  • Suspense ledger: revenue waits for documents, never faith

Revenue software that cannot show its evidence is an audit finding waiting to happen. Every dollar Someris suggests ships with the chain that survives the audit.

[04] Submit

Validated before it leaves the building.

The assessment goes out the door already checked: CMS item-consistency edit rules cleared, the HIPPS code recomputed against the CMS grouper logic itself, and the submission XML in the exact format the live CMS validator has already accepted. Submission snapshots are immutable, so what was filed is always answerable.

  • iQIES MDS 3.0 XML generation, current flat format
  • 330+ CMS edit rules run before submission
  • HIPPS recompute audited against CMS Grouper V2.4000
  • Submission gate: no finalization until checks pass
  • Immutable submission snapshots and billing export

Everyone claims CMS compliance. Ours is checked against the CMS validator before anything leaves the building.

[05] Defend

The audit answer already exists.

When the ADR letter, the surveyor, or the ransomware arrives, the response is already built. Every item carries its pre-assembled evidence packet. Every access to patient data is logged and retained. Backups live in an isolated vault under a locked retention policy that even our own administrators cannot override, and the restore path is drilled with a stopwatch, not a policy document.

  • Per-item evidence packets, ready for ADR and denial response
  • Audit-risk and survey-readiness surfaces
  • AES-256 encryption, access logging, 7-year audit retention
  • Isolated backup vault with locked 30-day retention
  • Scripted, timed restore drills
  • Ransomware tabletop exercise on file

Compliance as a marketing layer versus compliance you can drill. The restore path is rehearsed, not assumed.

[06] Sovereign

Your model is yours. Provably.

Every client owns the model tuned on their facility and can leave with it: weights, training manifest, evaluation harness, all signed and hash-verified. A tuned model is promoted only if it beats or ties the previous one on that client's own audited assessments, with zero payment-item regression. Before any model ships, we attack it ourselves with a membership-inference audit, and a failing privacy score blocks promotion unconditionally.

  • Per-client models, client-owned, exportable on exit
  • No-regression promotion gates on client gold data
  • Membership-inference privacy self-audit on every promotion
  • Cryptographic signing of every promoted artifact
  • De-identification pipeline with canary verification
  • Append-only training-data provenance manifest

The anti-lock-in that locks in: clients stay because leaving is easy and staying compounds.

About performance numbers: Someris publishes none until its pre-registered superiority trial reports. Every figure it will publish cites a scorecard snapshot from that frozen analysis, states the corpus size, and names the adjudication method. Internal engineering measurements stay internal until then; nothing is projected, modeled, or presented as "up to". Someris provides clinical decision support; a licensed clinician reviews and attests every assessment.
